Job summary

Doctors Hospital of Manteca seeks an experienced healthcare supervisor to drive case management excellence and optimize patient outcomes. You will oversee care coordination, assess utilization, and guide discharge planning across multidisciplinary teams.

The role requires a California RN license with BLS and a minimum of five years in acute care plus three years in case management. Per diem with rotating shifts supports patient needs and hospital operations.

Qualifications

• A.S. or equivalent preferred.
• B.A./B.S. preferred.
• Valid California RN license and Healthcare Provider BLS.
• 5+ years acute care experience required.
• 3+ years case management experience required.

Responsibilities

• Monitor delivery of care for patients identified by case finding criteria or referrals.
• Identify variances and help develop strategies to reduce variance patterns.
• Coordinate discharge planning and social/chaplain services as needed.
