Job summary

Doctors Hospital of Manteca seeks an experienced healthcare supervisor to drive case management excellence and optimize patient outcomes. You will oversee care coordination, assess utilization, and guide discharge planning across multidisciplinary teams.

The role requires a California RN license with BLS and a minimum of five years in acute care plus three years in case management. Per diem with rotating shifts supports patient needs and hospital operations.

Qualifications

• A.S. or equivalent preferred.
• B.A./B.S. preferred.
• Valid California RN license and Healthcare Provider BLS.
• 5+ years acute care experience required.
• 3+ years case management experience required.

Responsible for monitoring the delivery of care to patients identified by case finding criteria and/or referral for case management services. Identifies variances and assists in the development of strategies to reduce variance patterns. Ensures appropriate utilization of facilities and services and provides for the review of admission, continued stay and professional services rendered. Coordinates discharge plan and needed social service and/or chaplain services. Ensures planning to facilitate patient’s return to their optimum level of function. Performs data collection as indicated for Commitment to Quality. Performs other duties as required.

EDUCATION:

A.S. OR EQUIVALENT PREFERRED

B.A./B.S. Preferred

LICENSES / CERTIFICATIONS:

Valid California RN license and Healthcare Provider BLS.

EXPERIENCE:

5+ YEARS (acute care experience required) AND
3+ YEARS (Case Management experience required)
