Dakota Home Care in Fargo, ND is seeking a Nurse Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness across the full patient care cycle.

Responsibilities include assessing, planning, implementing, monitoring and evaluating actions to meet health and human service needs; coordinating care with the medical team; and advocating for clients to inform decisions and improve outcomes.

Qualifications

• Bachelor's degree required for nursing background.
• RN license is required and must be current.
• Driver's license required to fulfill community visits.
• Auto insurance and vehicle registration required.

Responsibilities

• Coordinate and provide safe, timely, effective patient care.
• Handle case assignments, draft care plans, review progress, and determine closure.
• Help clients achieve wellness and autonomy.
• Facilitate case coordination and information sharing with care teams.
• Advocate for patients regarding clinical status and treatment options.
• Develop working relations with medical staff throughout the care cycle.
• Record case information and complete forms accurately.
• Promote quality, cost-effective interventions and outcomes.
• Assess motivational and psychosocial goals; adhere to agency/state rules.
