Job summary

Dakota Home Care is seeking a Nurse Case Manager in the United States to enhance patient management, maximize satisfaction, and promote cost effectiveness. You will be responsible for the full patient care cycle, including assessment, planning, implementation, monitoring, and evaluation of care plans.

The role emphasizes coordinating safe, timely care while collaborating with medical teams and advocating for patients to support wellness and autonomy.

Qualifications

• At least 3 years clinical experience as an RN.
• Excellent organizational and time management skills.
• Problem solving skills and ability to multi-task.
• Ability to work as a team and collaborate with both office staff and community partners.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
• Handle case assignments, draft care plans, review case progress and determine case closure
• Help clients achieve wellness and autonomy
• Facilitate multiple care aspects (case coordination, information sharing, etc)
• Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
• Develop effective working relations and cooperate with medical team throughout the entire case management process
• Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
• Record case information and accurately complete all necessary forms
• Promote quality and cost-effective interventions and outcomes
• Assess and address motivational and psychosocial goals
• Adhere to professional standards as outlined by agency protocols and state rules and regulations
