ComForCare is building a brand-new home health agency in South Sarasota, FL. We are seeking a Florida-licensed RN to perform in-home assessments, develop care plans, and support families while ensuring compliance and quality care.

Flexible/part-time scheduling provides meaningful patient contact and community-based nursing opportunities. Responsibilities include comprehensive assessments, care plan updates, caregiver training, and inter-agency collaboration to ensure safe, effective home health

Qualifications

Responsibilities

• Conduct comprehensive in-home nursing assessments for clients.
• Evaluate health status, functional needs, safety concerns, and care requirements.
• Develop and update individualized care plans based on assessment findings.
• Determine caregiver support and service needs.
• Perform reassessments when conditions change.
• Communicate assessment findings with clients, families, and the ComForCare team.
• Collaborate with the office to implement care plans.
• Identify changes in condition and follow up on concerns.
• Provide clinical guidance and education to caregivers and families.
• Maintain compliant clinical documentation.

Flexible/Part-Time

Assessment visits are scheduled based on client needs and availability. This position may be an excellent fit for an RN seeking supplemental income, greater schedule flexibility, or a way to transition into community-based nursing.

What You'll Do

• Conduct comprehensive in-home nursing assessments for new and existing clients.
• Evaluate clients' health status, functional needs, safety concerns, and care requirements.
• Develop and update individualized care plans based on assessment findings.
• Determine appropriate caregiver support and service needs.
• Conduct reassessments when a client's condition or care needs change.
• Communicate assessment findings and recommendations with clients, families, caregivers, and the ComForCare team.
• Collaborate with the office team to ensure care plans are accurately communicated and implemented.
• Identify changes in condition, potential risks, and other concerns requiring follow-up.
• Provide clinical guidance and education to caregivers and families as appropriate.
• Maintain accurate, timely, and compliant clinical documentation.
• Develop QAPI.
• Train all new caregivers.
• Follow all applicable Florida regulations, agency policies, and professional nursing standards.
• Support ComForCare's commitment to providing compassionate, person-centered care.

What We're Looking For

• Florida RN license in good standing

Preferred Qualifications

• 2+ years of nursing experience, including home health, home care, hospice, geriatrics and/or community-based
• Skilled nursing visits or home health documentation experience
• Familiarity with Florida home health regulations (Ch. 400, F.S.; Ch. 59A-8, F.A.C.)
• Reliable transportation required and ability to travel to patient homes throughout the service territory; daily local travel required within Sarasota County (Englewood, Osprey, Nokomis, Venice, North Port).
