ComForCare Home Care in Broward County is hiring a part-time Registered Nurse to provide in-home nursing services. The role supports clients across Broward County with a flexible schedule, reporting to the Clinical Supervisor/Director of Clinical Services.

RN licensure in Florida is required, with preferred BSN and dementia care experience. The position emphasizes thorough assessments, care planning, and documentation to ensure quality patient outcomes while maintaining safety and compliance

Qualifications

  • Graduate of an accredited school of professional nursing; RN license in Florida (required).
  • Current Florida RN license.
  • Minimum of two years' home health experience preferred.
  • Experience with dementia/Alzheimer’s patients strongly preferred.
  • Licensed driver with insured vehicle and good driving record.
  • Current CPR certification.
  • Strong communication and interpersonal skills.

Responsibilities

  • Assess client status and develop an individualized plan of care.
  • Provide skilled nursing interventions to achieve client outcomes.
  • Document observations and adjust the care plan as needed.
  • Communicate changes to physicians and the care team promptly.
  • Maintain client records and participate in team meetings.

POSITION SUMMARY

Provides services to clients in accordance with the State Nurse Practice Act, Agency policy, and accepted professional standards of practice.


The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.


REPORTS TO: Clinical Supervisor/Director of Clinical Services


QUALIFICATIONS


  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.

  • Current license as a Registered Nurse in the state of Florida.

  • Minimum of two years of home care experience preferred.

  • Experience with dementia/Alzheimer’s patients strongly preferred.

  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.

  • Current CPR certification.

  • Strong communication and interpersonal skills.


ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY


  1. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.

  2. Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

  3. Develops and/or follows an individualized Plan of Care.

  4. Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period.

  5. Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).



  1. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:

  2. Teaching skills according to the client's needs and within the context of the client's environment, value system, and physical/emotional status.

  3. Medication administrations.



  1. Communicates effectively and in accordance with Agency requirements.

  2. Reports significant changes in client status to physician and other members of the team in a timely manner consistent with client needs.

  3. Maintains client records showing systematic assessment, planning intervention, and evaluation.

  4. Participates in team meetings and clarifies individualized reporting responsibilities with all new assignments.

  5. Documents legibly and according to Agency documentation guidelines and standards.

  6. Follows Agency guidelines for completion of forms, reflecting the orders and care plan.


  • Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. Initiates referrals as needed to other health team members when delegated by the Case Manager.

  • Promotes personal safety and a safe environment for clients and coworkers.


    1. Demonstrates knowledge of safety/infection control practices by complying with established policies and procedures.

    2. Recognizes and responds appropriately to potentially unsafe situations.

    3. Demonstrates safe practice in the use of equipment.

    4. Assesses environmental safety and takes initiative to prevent accidents and promote safety.

    5. Notifies supervisor of need for review and/or training related to equipment, procedures, or safety/infection control practices.



  • Participates in ongoing, professional self-development.


    1. Participates in all mandatory education per Agency guidelines.

    2. Identifies needs for personal professional growth.

    3. Demonstrates competency in areas of assignment or identifies the need for additional training as appropriate.


  • Supervises applicable home care team members as delegated by RN Case Manager.

  • Practices nursing with respect for individual, cultural, and spiritual differences.

  • Maintains confidentiality in all aspects of the job.

  • Performs other duties as assigned.
