Job summary

AMN Healthcare is seeking an RN Case Manager to coordinate the patient care continuum from admission to discharge in Pasadena, TX. You will work with physicians, nurses, and external providers to align treatments with recovery goals and ensure safe transitions.

Qualifications

• TX/compact RN license
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break

Responsibilities

• Coordinate patient care from admission through discharge.
• Collaborate with physicians, nurses, and external providers to remove barriers to care.
• Support safe, efficient discharge planning.
