Saint Francis Hospital, part of Trinity Health Of New England, is seeking an experienced Registered Nurse Case Manager for day shift. This role coordinates patient care across the continuum to ensure safe, high-quality, and cost-effective outcomes, collaborating with physicians, the inter-disciplinary team, patients, and families.

You will assess needs, develop and implement care plans, monitor progress, and facilitate timely transitions of care while advocating for patients’ well-being.

Qualifications

• Graduation from an accredited nursing school.
• Valid RN License in Connecticut (CT).
• Bachelor's degree in Nursing preferred.
• 3 years of acute care RN experience preferred.
• Knowledge of HCFA/OBRA and Joint Commission standards.

Responsabilités

• Coordinate care across the interdisciplinary team.
• Assess clinical, psychosocial, and discharge needs.
• Monitor patient progress and outcomes.
• Facilitate timely transitions of care.
