Job summary

Seva Hospice in Visalia, CA is seeking an RN Case Manager to join our compassionate team, providing care and support to terminally ill patients. This full-time role includes on-call weekend rotation and serves Tulare County and surrounding communities.

You must be a licensed RN with CPR certification, strong communication, and a patient-centered approach. Responsibilities include symptom management, emotional support, and coordinating care to help patients live their final days with dignity.

Qualifications

• Must be a registered nurse with active CA RN license.
• 1 year hospice or nursing experience preferred.
• Current CPR certification required.
• Good interpersonal and compassionate care skills.
• Reliable transportation and ability to participate in on-call weekends.

Responsibilities

• Provide direct care to terminally ill patients in their homes or facilities.
• Perform symptom management and adjust care plans as needed.
• Offer emotional support to patients and families.

Seva Hospice is a locally owned and operated company of experienced hospice providers with a desire to serve the communities we live and work in. Due to growth, we are looking for an RN Case Manager to join our dynamic team to provide care and support to terminally ill patients. This is a full time position, participating in on call weekend rotation. This position serves Tulare County and surrounding communities.

Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in caring for the terminally ill, providing expert symptom management and providing emotional support. The goal is to help patients live their final days with dignity and comfort.
