Job summary

Home Care with Heart is seeking a Registered Nurse Case Manager to provide home-based nursing care in the Warren, OH area. The role involves assessing and coordinating care with a multidisciplinary team to support patients in their homes.

Responsibilities include care planning, medication administration, and maintaining accurate clinical notes, with opportunities for on-call rotation and ongoing education.

Qualifications

• RN license and CPR certification required.
• Six months’ home care experience preferred.
• Nursing supervisory/leadership experience preferred.

Responsibilities

• Deliver comprehensive nursing care in the home care setting.
• Assess, plan, direct and evaluate each patient’s care plan.
• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers.
