ExcelaCare is seeking a Registered Nurse to deliver skilled nursing care in patients' homes, coordinating with a multidisciplinary team and families. The role requires independent practice with adherence to policy and plan of care.
You will administer treatments, document progress, educate patients, and participate in ongoing education while maintaining compassionate, patient-centered care in diverse home environments.

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with a multi-disciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care. Works independently without on-site supervision.
Treats all information and data within the scope of the position with appropriate confidentiality and security
Education: Graduate of an accredited school of nursing
Experience: One year of experience in nursing
License / Certification: Current State RN license; current CPR; current driver’s license and proof of auto liability
Works in patient’s home in various conditions, possible exposure to blood and body fluids and infectious diseases. Ability to work flexible schedule, ability to travel locally, some exposure to unpleasant weather. Moderate noise level; tasks may involve moderate stress and emotional demands.
Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and resource acquisition; meet patient/family psychosocial needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.
