Job summary

ExcelaCare Hospice in Roswell, NM seeks an experienced RN Case Manager for full-time, serving patients at home or facilities. The role coordinates care with a multidisciplinary team, develops and implements individualized plans of care, and educates patients and families on end-of-life options.

A $5,000 sign-on bonus is offered and travel to local patients is required. The RN will work independently with Excellence in compassionate hospice care and maintain compliance with agency policies and

Qualifications

• Graduate of an accredited school of nursing.
• Two years of RN experience in a clinical setting; hospice preferred.
• Current valid State Nursing license (RN) and CPR certification.
• Driver’s license and proof of auto liability insurance; no OIG exclusion.
• Ability to travel locally to patient locations.

Responsibilities

• Administers hospice care to patients in their homes or facilities.
• Coordinates care with a multidisciplinary team and family.
• Develops and implements a written Plan of Care per physician orders.
• Educates patients and families on care plans and end-of-life options.
• Provides independent nursing duties with on-call rotation as required.
