Episcopal Homes' Beyond Hospice is seeking an experienced Registered Nurse Case Manager to provide end‑of‑life care for hospice patients in Oakdale, MN. The role emphasizes independent practice within a strong, interdisciplinary team focused on comfort and quality of life.

Salary ranges $37.44–$56.66 per hour with day schedule, Monday–Friday, 8:00 a.m.–4:30 p.m., and every 6th weekend rotation. Excellent benefits, professional development, and campus amenities accompany this full‑time

Qualifications

• Current Minnesota Registered Nurse license is required.
• Graduate of an accredited nursing program.
• Strong assessment, communication, critical-thinking, and documentation skills.

Responsibilities

• Manage and coordinate care for an assigned group of hospice patients.
• Complete nursing assessments and individualized plans of care.
• Provide skilled nursing care focused on comfort, symptom management, and quality of life.
• Educate and support patients, families, and caregivers throughout the hospice journey.
• Coordinate medications, equipment, supplies, and community resources.
• Collaborate with physicians, social workers, chaplains, therapists, and other hospice team members.
• Communicate changes in condition and obtain provider orders as needed.
• Maintain timely and accurate clinical documentation.
• Participate in interdisciplinary team meetings and on-call rotation.

Pay & Schedule

Pay: $37.44–$56.66/hour, based on experience

Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.

Weekend Rotation: Every 6th weekend

What You’ll Do

Qualifications

Required

• Current Minnesota Registered Nurse (RN) license
• Graduate of an accredited school of nursing
• Strong assessment, communication, critical-thinking, and documentation skills
• Valid driver’s license, reliable transportation, and current auto insurance

Preferred

• Bachelor’s degree in Nursing (BSN)
• Two years of acute care nursing experience
• Two years of recent hospice or home health experience
