Job summary

Beyond Hospice, a service of Episcopal Homes of Minnesota, in Oakdale, MN, is seeking an experienced Registered Nurse Case Manager to deliver compassionate end-of-life care for patients and families. You will work independently within a supportive interdisciplinary team to coordinate plans and resources.

Qualifications

• Current Minnesota RN license.
• Graduate of an accredited nursing program.
• Two years of acute care nursing experience.
• Two years of hospice or home health experience.

Responsibilities

• Manage and coordinate care for an assigned group of hospice patients.
• Complete nursing assessments and individualized plans of care.
• Provide skilled nursing care focused on comfort, symptom management, and quality of life.
• Educate and support patients, families, and caregivers throughout the hospice journey.
• Coordinate medications, medical equipment, supplies, and community resources.
• Collaborate with physicians, social workers, chaplains, therapists, and other members of the hospice team.
• Communicate changes in condition and obtain provider orders as needed.
• Maintain timely and accurate clinical documentation.
• Participate in interdisciplinary team meetings and the on-call rotation.

Beyond Hospice, a service of Episcopal Homes, is seeking an experienced and compassionate Registered Nurse Case Manager to provide meaningful end-of-life care and support to patients and their families. This is a great opportunity for an RN who enjoys independence, relationship-centered care, and being part of a supportive interdisciplinary team.

Pay & Schedule

• Pay: $37.44–$56.66/hour, based on experience
• Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.
• Weekend Rotation: Every 6th weekend

What You’ll Do

Qualifications

• Current Minnesota Registered Nurse (RN) license
• Graduate of an accredited school of nursing
• Strong assessment, communication, critical-thinking, and documentation skills
• Valid driver’s license, reliable transportation, and current auto insurance
• Preferred Bachelor’s degree in Nursing (BSN)
• Two years of acute care nursing experience
• Two years of recent hospice or home health experience
