Job summary

Bristol Hospice in Oakdale, MN is seeking an RN Case Manager to support patients and families through the end-of-life journey. You will develop care plans, monitor conditions, and coordinate with a compassionate care team.

The role requires an active RN license, CPR certification, strong communication, and a valid driver’s license for patient visits. Monday–Friday schedule with day shifts, competitive salary, and comprehensive benefits.

Qualifications

• Must hold a valid RN license in the state where you work.
• CPR certification is required.
• Strong communication and observation skills are essential.
• Driver's license and reliable transportation required.

Responsibilities

• Develop and implement individualized care plans for patients.
• Provide one-on-one support to patients and families.
• Assess physical, emotional, and spiritual needs throughout care.
• Monitor vital signs and document findings.
• Administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
• Track changes in patient condition and respond promptly.
• Maintain detailed clinical notes and coordinate with care teams.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Coverage Area: Twin Cities

Work Schedule: Monday - Friday 8am - 5pm

Qualifications

On An Average Day You Will: (includes, but not limited to) Make a true difference—one patient, one family, one compassionate moment at a time.

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
