Job summary

Heart’n Soul Hospice in Florida seeks an experienced RN Case Manager to join a compassionate team delivering patient- and family-centered end-of-life care.

You will conduct comprehensive assessments, develop and revise the care plan, coordinate with the IDG, participate in care planning, and support on-call duties with timely documentation. A Florida RN license and a valid driver’s license are required.

Qualifications

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current State Registered Nurse License.
  • Valid driver's license with reliable transportation and auto liability insurance.

Responsibilities

  • Perform initial and ongoing comprehensive assessments of patient needs and ADLs in terminal illness.
  • Implement and revise the plan of care as needed.
  • Coordinate care with IDG members and healthcare professionals; participate in care planning meetings.

What You Must Have:
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State Registered Nurse License
  • Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
  • Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
  • Implement the individualized POC and recommend revisions to the plan as necessary
  • Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
  • Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
