Job summary

Community Care, Inc. in Wisconsin is seeking a full-time Case Manager to support elderly individuals and adults with physical or intellectual disabilities in the community. You will coordinate with an RN Case Manager and develop member-centered care plans to promote independence.

Responsibilities include assessments, care coordination, and ensuring documentation meets standards. Travel locally is required; schedule is Monday–Friday, 8:00 AM–4:30 PM. A $5,000 sign-on bonus is offered.

Qualifications

• Bachelor’s Degree in Human Services or closely related field required.
• Minimum of three (3) years’ experience with seniors or adults with physical or intellectual disabilities; non-human services degree may substitute.
• Eligibility for long-term care functional screener certification required.
• Regular local travel; valid driver’s license and insurance coverage required.

This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position located in Ozaukee County.

Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

Education: Bachelor’s Degree in a Human Services or closely related a rea required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.

Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred .

Certification: Eligibility for long-term care functional screener certification is required

·
Travel : Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
