Job summary

Homewatch Caregivers of St Cloud Minneapolis St Paul seeks an RN Case Manager to coordinate and deliver in-home nursing care. You will manage a caseload, perform clinical assessments, develop care plans, and liaise with care teams to ensure seamless patient progress.

The role includes hands-on nursing tasks in clients' homes, wound care, vital signs, and discharge planning, with strong emphasis on documentation and communication with families and providers.

Qualifications

• RN license in Minnesota (required).
• BLS certification & CPR required.
• Oasis experience not mandatory but preferred.
• Strong clinical skills and ability to conduct comprehensive assessments.
• Minimum one year of nursing experience (home health or hospice preferred).
• Exceptional communication and interpersonal abilities.

Responsibilities

• Perform comprehensive assessments and maintain individualized care plans.
• Coordinate with providers and supervise unlicensed staff.
• Complete reauthorizations and maintain accurate documentation.
• Provide health education and support to clients and families.
• Deliver hands-on nursing care (vital signs, MARS/TARS, discharge planning, etc.).

Includes field nursing tasks such as wound care, glucose checks, and medication setup.

Minimum Requirements:

• Valid and unrestricted Nurse (RN) license in the State of Minnesota (required)
• BLS certification & CPR (required)
• Oasis experience not mandatory but preferred
• Strong Clinical skills and the ability to conduct comprehensive assessments
• Minimum one year of nursing experience (home health or hospice preferred)
• Exceptional communication and interpersonal abilities
