Job summary

Essentia Health is seeking a registered nurse to coordinate and deliver patient care within the Case Management and Social Services team. The role focuses on assessment, planning, intervention and evaluation to promote optimal outcomes and discharge readiness.

You will collaborate with physicians and a broad clinical team to ensure quality care, efficient resource use, and effective communication with payers. A BSN/ADN with active RN licensure is required.

Qualifications

• BSN or ADN from an accredited school.
• Current RN licensure in the appropriate state(s).
• BCLS certification within 1 month of start date.

Responsibilities

Building Location: Building B - St Marys Medical Center Department: 2005320 CASE MGMT-SOCIAL SERVICES - SMMC HOSP Job Description: Responsible for organizing and providing care to patients through the process of assessment, planning, intervention, and evaluation. Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. This role is accountable for planning effectively in order to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload.
