Job summary

St. Croix Hospice in Wisconsin is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in their homes, coordinating care plans with an integrated care team.

The role requires an RN with a nursing degree, at least 1 year in acute care, hospice or home health preferred, and willingness to on-call; the position offers competitive salary and comprehensive benefits, including health, dental, vision, life insurance, 401(k) with match, PTO and holidays.
