Job summary

Bristol Hospice in Sun Prairie, WI is seeking an experienced RN Case Manager to guide patients and families through every step of the hospice journey. This full-time role offers a Monday–Friday schedule, with a competitive salary range and comprehensive benefits.

You will develop individualized care plans, perform thorough assessments, and collaborate with interdisciplinary teams to deliver compassionate, high-quality end-of-life care.

Qualifications

• Active RN license in good standing in the state you will work in.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong observational and written/verbal communication skills.
• Flexible, team-oriented attitude and willingness to support additional duties.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track changes in patient condition and respond timely.
• Maintain up-to-date clinical notes reflecting patient progress.
• Collaborate with interdisciplinary teams for comprehensive care.

Qualifications

• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
