Job summary

Bristol Hospice - Madison LLC in Sun Prairie, WI is seeking an experienced RN Case Manager to support patients and families with compassionate, individualized end-of-life care. This full-time role offers a Monday–Friday schedule, a salary range of $85,000 to $98,000, and a culture built on integrity, respect, and teamwork within a mission-driven hospice environment.

Qualifications

• Valid RN license in the state you will work in.
• Valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills, written and verbal.
• Flexible, team-oriented attitude and willingness to support duties as needed.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort and dignity.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor vital signs (temperature, pulse, respiration, blood pressure).
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely responses.
• Keep detailed clinical notes reflecting patient progress and care quality.
• Collaborate with interdisciplinary teams for comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate team.

Qualifications

What You'll Bring to The Team:

• A valid Registered Nurse license in good standing in the state you will work in
