Job summary

Bristol Hospice Enterprise, AL is seeking a Registered Nurse Case Manager to provide compassionate, whole-person hospice care and advocate for patients and families. This full-time role requires an active RN license and CPR certification, with reliable transportation and a strong communication skill set.

You will deliver individualized care plans, monitor patient status, and collaborate with interdisciplinary teams to ensure high-quality end-of-life care within our mission-driven culture.

Qualifications

• Valid Registered Nurse license in good standing in the state you will work in.
• Valid driver's license and reliable transportation.
• CPR certification (current).
• Strong observational and written/verbal communication skills.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

On An Average Day You Will:

• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
