Cynet Health is seeking an RN Case Manager for a 13-week assignment in Oregon. The role is day shift, 8:00 AM - 5:30 PM, with a minimum of 2 years RN experience in case management. An active RN license and BLS certification are required.

The hospital is a state-of-the-art, 42-bed inpatient acute rehabilitation facility focusing on helping patients regain independence after injury or illness. IRF and Med-Surg experience are highly valued, and EPIC EHR proficiency is mandatory.

Competenze

Description:This position is available for an immediate start.Candidates must possess an active RN license.No local candidates will be considered.Candidates without long-term care experience or new graduates will not be eligible.The facility is a state-of-the-art, 42-bed inpatient acute rehabilitation hospital.It focuses on helping individuals regain independence after injury or illness.Rehabilitation programs provide ongoing care and specialized treatment tailored to individual needs.Common conditions include stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.The goal is to maximize health, function, and quality of life through comprehensive physical medicine and rehabilitation.The position requires working within a patient ratio of 5:1 for RN/LPN and 7:1 for CNA.Experience with EPIC EHR is mandatory.The patient population primarily consists of adults, with some geriatric and rehabilitation patients.Weekend rotation is required every other week.There are no floating requirements for this position.
