Moments Hospice is seeking an experienced RN Case Manager to serve as the clinical lead for patients and families in Wisconsin. You will manage symptoms, coordinate care plans, and guide families through the final chapter of life with compassion.

This full-time, field-based role offers a competitive salary of $75,000 to $90,000 per year, a supportive team, and direct access to leadership as you shape your hospice career.

Pay: $75,000 - $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday-Friday, 8:30 AM - 5:00 PM
