Moments Hospice in Omro, WI is seeking an RN Case Manager to serve as the clinical lead for patients and families. This full-time, field-based role involves managing symptoms, coordinating care, and providing compassionate guidance during the final chapter of life.

RN Case Manager - Hospice

Pay: $75,000 - $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday-Friday, 8:30 AM - 5:00 PM
