Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in a hospice setting. You will manage symptoms, coordinate services, and provide guidance during end-of-life care.

The role emphasizes team collaboration, quality care, and supporting families with education and compassion while meeting workload expectations. Fleet car provided for business and personal use where eligible.

Qualifications

• Active RN license in the state of Wisconsin.
• Proficiency with electronic medical records (EMR).
• Strong organizational and communication skills.
• Ability to work independently while maintaining high care standards.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care with patients and families.
• Provide skilled nursing care including pain and symptom management and medication administration.
• Educate patients and caregivers on disease progression and end-of-life expectations.
• Coordinate care across caseload and act as primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group meetings to ensure holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Responsibilities:

• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life

• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Qualifications:

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
