Job summary

Seva Home Health is seeking an RN Case Manager to cover Tulare County and surrounding communities. This per diem role focuses on one-to-one patient care in home settings, with a goal of compassionate, high-quality service.

The position offers competitive pay, benefits with medical, dental and vision, flexible scheduling, and 401k matching. Travel to patient homes is required, and prior home health/palliative care experience is preferred.

Qualifications

• Minimum of 1 year of experience as a Registered Nurse with a valid license.
• Current CPR/BLS Certification.
• Reliable transportation required.
• Experience with Kinnser strongly preferred.
• Comfortable driving to patient homes for patient visits.
• Prior experience with Palliative Care preferred.
• Prior experience in Home Health, Hospice, Infusion, or Home Care preferred.

Responsibilities

• Initiate, develop, and implement the patient Plan of Care (POC).
• Revise POC in consultation with the patient, physician, and other team members when appropriate.
• Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.
• Oversee and deliver quality, patient-centered home care.
• Communicate necessary revisions to patient status with the care team.
• Document promptly and accurately per the plan of care.
