Job summary

CareOne Home Health is seeking an RN Case Manager to cover Calaveras and Amador Counties and surrounding communities. The role focuses on high-quality, compassionate care delivered in patients' homes with training provided for home health protocols.

Requirements include an active RN license in California, CPR/BLS certification, and at least 1 year of RN experience. Home health experience is preferred, with Kinnser experience strongly preferred for charting and coordination.

Qualifications

• Must hold active RN license in California.
• Current CPR/BLS certification required.
• Minimum of 1 year of experience as a Registered Nurse.
• Home health experience preferred; training provided.

Responsibilities

• Initiate, develop, and implement the patient Plan of Care (POC).
• Revise as needed with patient, physician, and care team.
• Oversee delivery of quality, patient-centered home health care.
• Communicate revisions to patient status with the care team and family.
• Regularly reevaluate patient status and assist with discharge planning.
• Provide complete and timely documentation aligned with the POC.

Revise as needed in consultation with the patient, physician, and other team members when appropriate.Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility.Communicate necessary revisions to patient status and/or needs with the entire care teamRegularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.Provide complete, accurate and timely documentation consistent with the plan of care
