Job summary

Seva Home Health seeks an experienced RN Case Manager to cover Madera and Fresno Counties. The role focuses on delivering high-quality, compassionate home-based care and coordinating with physicians and families.

The ideal candidate has 1+ year RN experience, CPR/BLS, and is comfortable driving to patient homes. Training and support are provided to ensure excellent patient outcomes.

Qualifications

• Active Registered Nurse license required.
• Minimum 1 year of RN experience with valid license.
• Experience with Kinnser EMR strongly preferred.
• Home health, hospice, infusion, or home care experience preferred.

Responsibilities

• Develop and implement the patient Plan of Care (POC) and revise as needed with interdisciplinary team.
• Set patient-specific goals and provide therapeutic, preventative and rehabilitative nursing services.
• Oversee delivery of quality, patient-centered care in patient homes or living facilities.
• Communicate revisions to patient status with care team and family as appropriate.
• Provide timely, accurate documentation aligned with the plan of care.
