Job summary

AMN Healthcare is seeking an RN Case Manager in New York City for a 13-week assignment. You will coordinate discharge planning from inpatient units, ensuring smooth post-acute arrangements and effective communication with patients and families.

The role requires NY RN license and at least 5 years of acute care case management experience; hospice knowledge is a plus. Shifts are 7.5 hours daily, Monday through Friday, with Epic as the primary system.
