Job summary

Bristol Hospice is seeking dedicated RN Case Managers to advocate for patients and families, guiding them through hospice care with compassion and dignity.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Deliver attentive, one-on-one support to patients and families.

• Monitor and document vital signs and patient progress accurately.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
