Job summary

RN.com is seeking a skilled RN Case Manager in the Los Angeles area to oversee care coordination, discharge planning, and patient-centered care planning in a dynamic hospital setting.

The position requires an active CA RN license, BLS, and experience with hospital care coordination, ideally within CommonSpirit/Dignity networks. Assignment runs 13 weeks with 8-hour days, 40 hours per week, on-site in Los Angeles.

Qualifications

• CA RN license required.
• BLS and LA City Fire & Safety certifications required.
• At least 2 years of acute hospital care coordination experience; prior CommonSpirit/Dignity experience.
