Job summary

RN.com in Detroit is seeking an experienced RN Case Manager to coordinate patient care, assess needs, and develop individualized discharge plans. You will collaborate with interdisciplinary teams and utilize Cerner to ensure appropriate resource use for optimal recovery and transitions.

Minimum qualifications include a Michigan RN license, BLS, and at least 1 year of case management experience in an acute hospital setting. The assignment runs for 13 weeks with a Monday–Friday schedule.

Qualifications

MINIMUM REQUIRED QUALIFICATIONS

• MI RN license
• BLS
• 1+ year case manager experience in an acute hospital setting

PREFERRED QUALIFICATIONS

– Acute rehab experience

LENGTH OF ASSIGNMENT

– 13 weeks

SHIFT / HOURS PER WEEK

– M-F 8:00am-4:30pm
