Job summary

CareOne Home Health is seeking an experienced RN Case Manager to cover Monterey County and surrounding communities. The role focuses on high-quality, compassionate home health care in patients' homes with training and support provided. Per diem with potential for full time as we grow.

The ideal candidate has at least 1 year of RN experience, current CPR/BLS, and reliable transportation to visit patient homes. Home health experience and Kinnser software familiarity are strongly preferred.

Qualifications

• RN license is required
• 1+ year of RN home health or related setting preferred
• CPR/BLS certification required
• Reliable transportation and ability to drive to patient homes
• Experience with Kinnser home health software preferred
• Palliative care experience preferred
• Home health, hospice, infusion, or home care background preferred

Responsibilities

• Initiate, develop, and implement the patient Plan of Care (POC) in collaboration with the patient, physician, and care team
• Set patient-specific goals and coordinate therapeutic, preventative and rehabilitative nursing services
• Oversee and deliver quality, patient-centered care in patients' homes or living facilities
• Communicate revisions to patient status with the care team and patient/family
• Provide timely, accurate documentation consistent with the Plan of Care
