Job summary

Addus HomeCare is seeking an RN Case Manager for Tennessee Quality Care Hospice in Jackson. This field-based role manages patient caseloads, coordinates care, and supports families to deliver compassionate home-based nursing services.

The RN will conduct assessments, determine eligibility, plan and implement care, and collaborate with physicians and the interdisciplinary team to ensure quality outcomes for patients at home.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications currently valid.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of auto insurance

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data and assess factors that impact patient and family needs and initiate interventions with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans in a timely manner.
• Supervise and document delivery of care by Home Health Aide per guidelines

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Jackson. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory: Jackson, TN and surrounding counties

Schedule: Monday thru Friday 8am-4:30pm. Estimated on-call expectation 1 day a month.

Qualifications

• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver's license and proof of insurance is required

What You'll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
