Job summary

Tennessee Quality Care Hospice is hiring an RN Case Manager in Jackson, TN to manage a home-based caseload, provide therapeutic nursing, coordinate care, and support patients and their families in their homes.

The role covers patient assessment, care planning, and collaboration with an interdisciplinary team to ensure high-quality home hospice care. On-call duties are required about one day per month.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance.

Responsibilities

• Manage a patient and family caseload across the nursing process lifecycle.
• Assess data from patients and families and coordinate interdisciplinary care.
• Communicate with attending physicians and agencies to coordinate care.
• Provide regular patient visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support families.
• Maintain accurate records and care plans; supervise home health aide delivery.

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Jackson. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory:

Jackson, TN and surrounding counties

Schedule:

Monday thru Friday 8am-4:30pm. Estimated on-call expectation 1 day a month.

Qualifications:

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
