Job summary

Right at Home Boston Northwest & Cape Cod is seeking a compassionate Registered Nurse (RN) / Case Manager for our new Hyannis office. You will conduct in-home assessments, develop care plans, provide clinical management, and coach caregivers while building strong relationships with clients and healthcare partners.

We offer 24-40 hour schedules with flexibility, onboarding with nursing leadership, and the potential to take on broader clinical responsibilities as the office grows.

Qualifications

• Active, unrestricted Registered Nurse license in Massachusetts.
• Two or more years of nursing experience, preferably in home care or geriatrics.
• Strong clinical judgment, communication, organization, relationship-building, and documentation skills.

Responsibilities

• Conduct in-home assessments and reassessments and develop or update individualized care plans.
• Provide ongoing clinical case management, nursing supervision, follow-up visits, and timely documentation.
• Support, educate, and coach caregivers; respond to changes in condition and help ensure care plans are followed.
• Serve as a trusted clinical liaison for clients, families, caregivers, physicians, and other healthcare professionals.
• Partner with scheduling, care management, and operations teams to support staffing, continuity of care, and day-to-day operations.
• Build relationships with hospitals, rehabilitation centers, senior living communities, Aging Services Access Points, physicians, and other Cape Cod referral partners.
• Represent Right at Home professionally in the community and contribute to the growth of the new office.

At Right at Home Boston Northwest & Cape Cod, we are seeking a compassionate, organized, and relationship-focused Registered Nurse (RN) / Case Manager for our new Hyannis office. With support from our established operations team, you will coordinate high-quality care, support caregivers, build trusted relationships with clients, families, healthcare professionals, and community referral partners, and help establish the clinical foundation for the Cape Cod office.

Your Role as a Registered Nurse (RN) / Case Manager:

What We're Looking For:

• Active, unrestricted Registered Nurse (RN) license in Massachusetts.

• Two or more years of nursing experience; home care, geriatrics, private-duty care, or care-management experience strongly preferred.

• Strong clinical judgment, communication, organization, relationship-building, and documentation skills.

• A self-directed, collaborative professional who is comfortable taking initiative, adapting as the office grows, and developing strong referral relationships.

• Valid driver's license, insured vehicle, and ability to travel throughout Cape Cod.
