Job summary

St. Croix Hospice is hiring a Registered Nurse Case Manager to provide 1:1, whole-person care in patients’ homes. You’ll coordinate care plans with an integrated team, support families through complex decisions, and deliver care with empathy and professionalism.

Requirements include an accredited nursing degree (RN) and 1+ year acute care experience; hospice/home health preferred. On-call duties and travel may be required, with a competitive salary and benefits package.

• 1+ years of acute care experience; hospice/home health preferred.
• Strong assessment and care-plan coordination skills.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family and update as needed.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
