St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver in-home, compassionate care to patients and families. You will complete assessments, develop care plans, and coordinate with the care team to support dignified end-of-life journeys at home.

Qualifications

• Active RN license in good standing (any degree).
• 1+ year of acute care experience; hospice/home health preferred.
• Reliable transportation, valid driver's license, and auto insurance.

Responsabilités

• Conduct initial and ongoing patient assessments to determine hospice needs.
• Develop individualized care plans with families and update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and coordinate seamless care transitions.
