Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic home-based care. You will collaborate with an interdisciplinary team to create individualized care plans and guide end-of-life conversations with patients and families.

Responsibilities include assessments, medication administration, patient education, and coordinating on-call duties to ensure smooth transitions. A valid RN license and reliable transportation are required, with competitive benefits.

Qualifications

• RN licensed with an accredited program (Associates, Bachelors, or Masters).
• 1+ year of acute care experience; hospice/home health preferred.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Reliable transportation and valid driver’s license.

Responsibilities

• Develop individualized care plans with the patient and family, updating as needs evolve.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Administer medications and treatments as prescribed.
• Rotate on-call duties and ensure seamless care transitions.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
