St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care in patients' homes.

You will partner with an integrated care team to coordinate individualized care plans and support families through difficult decisions with compassion. The role requires an RN with an Associates, Bachelors, or Masters in Nursing and 1+ year acute care experience (hospice/home health preferred).

Qualifications

• 1+ years of acute care experience; hospice/home health preferred.
• RN licensure or eligibility required for role.
• Complete initial and ongoing assessments to determine hospice needs.
• Develop and update individualized care plans with patient and family.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Rotate on-call duties and manage seamless care transitions.
• Must have reliable transportation, valid driver’s license, and auto insurance.

An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
