Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person home care to patients and families. You will coordinate care plans with an integrated team, educate families, and support difficult decisions with empathy.

The role requires an RN with acute care experience and a commitment to compassionate, autonomous practice. On-call duties and transportation are part of the schedule.

Qualifications

• RN with accredited nursing degree (Associates, Bachelors, or Masters).
• 1+ years of acute care experience; hospice/home health preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
