Job summary

Community Healthcare of Texas is seeking an RN Case Manager for hospice and palliative care in Fort Worth, TX. You will meet with patients and families, coordinate admissions, and develop ongoing care plans.

Responsibilities include providing nursing care, evaluating patient status, administering medications, coordinating with the interdisciplinary team, and participating in on-call rotations to ensure quality end-of-life care. The role requires TX RN licensure and reliable transportation.

Qualifications

• Registered Nurse in the state of Texas.
• Valid driver’s license and reliable transportation.
• 1 year of nursing experience.
• Case management experience (hospice).
• 2+ years hospice case management preferred.
• Proficient computer skills; MS Office experience preferred.

Responsibilities

• Provide professional nursing care using the nursing process.
• Assess patient status and initiate a plan of care; re-evaluate with family needs.
• Develop and implement a hospice care plan with patient/family involvement.
• Administer medications and treatments as prescribed.
• Act as Case Manager coordinating patient care for the caseload.
• Document clinical notes and track pain/symptom changes.
• Communicate with physicians about patient needs and orders.
• Coordinate care with community health workers and hospice team.
• Maintain a safe environment for the patient.
• Participate in on-call rotation and assist with admits.
• Supervise LVN and CNA care plans when required.

Community Healthcare of Texas is currently recruiting for an RN Case Manager.

$5,000 Sign-on bonus

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate the patient’s status by writing and initiating a plan of care. Regularly re‑evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning process
• Administer medications and treatments as prescribed by the physician
• Provide healthcare instruction to the patient as appropriate per assessment and plan
• Act as Case Manager when assigned by the clinical Supervisor and assume responsibility to coordinate patient care for the assigned caseload
• Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition, obtain/receive physicians’ orders as needed
• Communicate with community health‑related persons to coordinate the care plan
• Provide and maintain a safe environment for the patient
• Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver
• Attend interdisciplinary group meetings
• Participate in on‑call rotation; may assist with admits
• Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis
• Supervise LVN and CNA care plans and delegate responsibilities when required.

QUALIFICATIONS

• Registered Nurse in the state of Texas
• Hold a valid driver’s license and have a dependable means of transportation
• 1 year of nursing experience
• Case Management experience (hospice)
• 2+ years of experience in hospice case management strongly preferred
• Proficient skills using a computer are required; Microsoft Office suite experience is preferred.
