Job summary

Ethos is seeking an experienced Hospice RN Case Manager in Dallas to deliver compassionate, patient-centered admission experience with clinical autonomy and supportive leadership.

You will evaluate prospective patients for eligibility, coordinate medications and services, communicate with physicians and families, and develop individualized plans of care in collaboration with the interdisciplinary team, ensuring timely, well-coordinated hospice care.

Qualifications

  • Current, unrestricted Texas RN license.
  • Minimum of one year of hospice nursing experience.
  • Knowledge of hospice eligibility and admission process.
  • Valid driver's license and reliable transportation.
  • Local residence near the position.

Responsibilities

  • Assess prospective patients to determine hospice eligibility.
  • Review medical history, symptoms, and functional status.
  • Communicate with physicians, referral sources, patients, families, and caregivers.
  • Explain hospice services, levels of care, and expectations clearly.
  • Lead families through difficult conversations with empathy.
  • Complete admission assessments and documentation promptly.
  • Develop individualized plans of care based on clinical condition and needs.
  • Coordinate medications, equipment, and services for timely transition.
  • Collaborate with Clinical Manager and team for smooth onboarding.

Job description

RN Hospice Case Manager This is an excellent opportunity for a hospice RN who values meaningful patient interaction, professional autonomy, supportive leadership, and a long-term future. The position works closely with clinical leadership, physicians, referral sources, and the interdisciplinary hospice team to ensure every patient receives a timely, compassionate, and well-coordinated admission experience. This position reports to the Clinical Manager, with additional leadership support from the Chief Operating Officer.

Position Responsibilities:
  • Evaluate and assess prospective patients to determine clinical eligibility for hospice services.
  • Review diagnoses, medical history, current symptoms, functional status, and supporting clinical documentation.
  • Communicate effectively with physicians, referral sources, patients, families, and caregivers throughout the admission process.
  • Explain hospice philosophy, services, benefits, levels of care, and patient and family expectations with clarity and compassion.
  • Guide patients and families through emotionally difficult conversations in a calm, respectful, and supportive manner.
  • Complete all required admission assessments, consents, documentation, and supporting clinical records accurately and promptly.
  • Initiate individualized plans of care based on the patient?s clinical condition, symptoms, goals, and immediate needs.
  • Coordinate medications, equipment, supplies, and other services necessary for a safe and timely transition into hospice care.
  • Collaborate with the Clinical Manager and interdisciplinary team to ensure seamless onboarding of every new patient.
Requirements
  • Current, unrestricted Texas Registered Nurse license.
  • Minimum of one year of hospice nursing experience.
  • Working knowledge of hospice eligibility requirements and the hospice admission process.
  • Valid driver?s license.
  • Reliable personal transportation.
  • Current automobile insurance.
  • Currently live locally to this position.
