Job summary

Pathways Hospice (Newest Start Up) seeks a compassionate Hospice RN Case Manager for a PRN role in the Greater Houston area. You will assess patients, coordinate care, educate families, and ensure documentation and regulatory compliance while advocating for patients' needs.

Candidates must hold an RN license in Texas (or a compact state) and have hospice experience. HCHB EMR familiarity is required, with a focus on interdisciplinary teamwork and crisis management.

Qualifications

• Bachelor’s or Master’s Degree in Nursing; coursework in palliative care and healthcare management is a plus.
• 1 year RN case management experience in hospice; prior nursing/hospice experience preferred.
• HCHB EMR experience is required or highly advantageous.
• Certifications in hospice and palliative care are advantageous.

Hospice RN Case Manager (PRN)

The Job:

A hospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey.

Responsibilities:

• Patient Assessment; Care Coordination; Direct Nursing Case; Education and Support; Documentation and Compliance; Advocacy

Must Haves:

• RN Registered Nurse in Texas or Compact state in good standing on license. A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous.
• Experience with RN Case Management in Hospice of at least one (1) year. Prior experience in nursing, particularly in hospice or palliative care settings is needed.
• Skills in crisis management and interdisciplinary teamwork are essential.
• Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management. Not required but a plus!
• Must have experience in HCHB EMR

Work Environment

• Hospice RN case managers often work in home health settings, providing care in patients' homes, which allows for a more personalized approach to end-of-life care.
