Pathways Hospice is seeking a Hospice RN Case Manager (PRN) to join our Southeast team. The role focuses on coordinating care for patients with terminal illnesses and supporting families through the end-of-life journey.

The position requires Texas RN licensure (or compact state), 1+ year in hospice case management, and strong teamwork. Experience with HCHB EMR is preferred; emphasis on compassionate, patient-centered care in home settings.

Qualifications

• Texas or Compact state RN license in good standing; hospice/palliative care certifications advantageous.
• Minimum 1 year RN case management experience in hospice or palliative care.
• Strong crisis management and interdisciplinary teamwork skills.
• Bachelor’s or Master’s Degree in Nursing; coursework in healthcare management is a plus.
• Experience with HCHB EMR is preferred.

Responsibilities

• Assess patients and coordinate individualized care plans.
• Coordinate care and direct nursing case management.
• Provide education and support to patients, families, and caregivers.
• Document care activities and ensure compliance with regulations.

The Job:

Ahospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey.

Responsibilities:

• Patient Assessment; Care Coordination; Direct Nursing Case; Education and Support; Documentation and Compliance; Advocacy

Must Haves:

• RN Registered Nurse in Texas or Compact state in good standing on license. A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous.
• Experience with RN Case Management in Hospice of at least one (1) year. Prior experience in nursing, particularly in hospice or palliative care settings is needed.
• Skills in crisis management and interdisciplinary teamwork are essential.
• Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management. Not required but a plus!
• Must have experience in HCHB EMR

Work Environment

• Hospice RN case managers often work in home health settings, providing care in patients' homes, which allows for a more personalized approach to end-of-life care.
