Job summary

The role involves assessing ED patients, identifying high‑risk readmission cases, coordinating discharge planning, and facilitating care transitions with the interdisciplinary team to improve patient outcomes.

Requirements include FL/compact RN license and BLS, plus 2 years of recent inpatient case management experience. Shift is 11a–7p.

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Assesses patients in the emergency department, identifies individuals at high risk for readmission, coordinates discharge planning and community resources, facilitates care transitions, and collaborates with the interdisciplinary team to reduce avoidable hospital readmissions and improve patient outcomes.

Minimum Required Qualifications –

• FL/compact RN license
• BLS
• 2 years recent case management experience in an inpatient acute hospital setting

PREFERRED QUALIFICATIONS – Care Management Certification (ACM Or CCM)
