Job summary

CenterCare Hospice is seeking a compassionate Registered Nurse to provide in-home hospice care in Pueblo, CO. The RN will assess patient and caregiver needs, coordinate care with the medical team, and educate families on disease progression and end-of-life care while ensuring compliance with all regulations.

The ideal candidate has an active RN license, CPR/BLS certification, and at least 1 year of hospice experience. Travel is required and a strong commitment to dignity and comfort is essential.

Qualifications

  • Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.
  • Must possess a current, unencumbered RN license.
  • Must have CPR/BLS certification and hospice experience preferred.

Responsibilities

  • Provide nursing care in patient homes in accordance with federal, state, and local standards and regulations.
  • Assess patient's and caregiver needs upon admission and throughout care.
  • Communicate effectively with physicians, care team, patients and families.
  • Educate patient and family about disease, self-care and end-of-life care.
  • Coordinate diagnostic and therapeutic services as ordered and ensure proper documentation.

Communicate to the care teamEducationMust possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN programSpecific RequirementsMust possess a current, unencumbered, active RN licenseMust have current CPR/BLSMinimum 1 year of Hospice experienceMust have reliable transportation and proof of auto insurance coverageMust pass a pre-employment background check and drug test
