Job summary

CenterCare Hospice in Pueblo, CO is seeking a compassionate Registered Nurse to provide in-home nursing care under federal, state, and local guidelines. The role emphasizes comfort, symptom management, and respectful, patient-centered support for patients and families.

The RN will assess patient needs, coordinate with the care team, educate families, and ensure accurate charting and compliance with policies. A 1-year hospice background and a current RN license are required.

Qualifications

• Must possess a current, unencumbered RN license.
• Must have current CPR/BLS.
• Minimum 1 year of Hospice experience.
• Reliable transportation and proof of auto insurance.
• Pass a pre-employment background check and drug test.

Responsibilities

• Provide nursing care in the patient’s place of residency per current standards and regulations.
• Determine the scope and frequency of services based on patient acuity and family needs.
• Assess caregiver ability to meet patient needs on admission and during care.
• Communicate with physicians and care team effectively.
• Educate patient and family about disease, self-care, and end-of-life care.
• Arrange diagnostic and therapeutic services as ordered by the physician.
• Follow drug administration protocols and proper disposal of meds.
• Perform charting per policies and regulations.
• Review care plans to ensure appropriate care is rendered.

RN- Job Summary:

Provide nursing care in the patient’s place of residency in accordance with current federal, state, and local standards, guidelines, and regulations that govern our organization, and as may be required by the Director of Nursing or designee.

RN- Essential Job Functions:

• Administer services within the applicable scope of nursing practice
• Determine the scope and frequency of services needed based on the acuity of the patient and the family/caregiver needs
• Assess the ability of the caregiver to meet the patient’s needs upon admission and throughout care
• Communicate with physicians, care team, patients and family members effectively
• Consult with and educate the patient and family/caregiver regarding the disease process, self-care techniques and end of life care
• Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures
• Follow drug administration protocol, to include the disposal of drugs and narcotics as required, and in accordance with established procedures
• Perform charting duties as required and in accordance with established charting and documentation policies and procedures and applicable state and federal regulations
• Review care plans to verify that appropriate care is being rendered. Communicate to the care team

QualificationsEducation

Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program

Specific Requirements

• Must possess a current, unencumbered, active RN license
• Must have current CPR/BLS
• Minimum 1 year of Hospice experience
• Must have reliable transportation and proof of auto insurance coverage
• Must pass a pre-employment background check and drug test
